You can also use the "Contact Me" block located at the bottom of my website pages. Just fill in the requested information, hit submit, and I will reach out to you after I have received your inquiry. I make every effort to respond to all requests within 24 hours.
For new clients, please do not contact me initially via text message. For all clients, as much as possible please only use text messaging for appointment changes or cancellations.
My session fee is $100.00 per 55-60 minute session. A 24-hour notice IS REQUIRED if you are unable to keep your scheduled appointment. You will be charged my FULL session fee of $100.00 for any missed or late cancellation appointment. (If you are using an insurance policy, please note that I cannot bill insurance for a missed appointment, and the $100.00 charge is applicable to you.) Payment for sessions is due at the time of your appointment and prior to the commencement of your session.
I accept as payment options cash, checks, or the following credit cards: Master Card, Visa, Discover.
I am currently a provider for some Blue Cross Blue Shield and Aetna insurance plans (I do not accept HMO plans). If you are using your health insurance policy for your sessions, your co-pay, co-insurance, or applicable deductible amount is your session fee. Please be sure you verify with your insurance carrier your payment due at the time of your session so you will not be unpleasantly surprised by an unanticipated billing. Please be aware that I MUST assign you a diagnosis code in order for your insurance carrier to consider my claim on your behalf. This diagnosis then becomes a permanent part of your medical records and anyone you release your medical records to will have access to this diagnosis information. Also, I file insurance on behalf of my clients as a courtesy. Payment for sessions with me is ultimately the full responsibility of the client.
